Destination Healthcare Services, is a residential medical care provider for adults with Intellectual and/or Developmental disabilities in Middle, Tennessee. We are a licensed Medicaid provider with a certification to operate a Medical Residential Facility in state of Tennessee. Destination Healthcare Services is certified by the Centers for Medicare & Medicaid Services (CMS).

Our Process
Destination Healthcare Services provides medical residential care facilities for Individuals with Intellectual Disablities (under 1915c waiver, and ECF choices currently) at homes that provide 24/7 support. Our homes maintain individual physical, intellectual, social, and emotional capabilities of people supported. Our homes are currently located in Davidson County, of Tennessee. Our homes meet all applicable federal, and state requirements for the individuals we service.
