Destination HealthCare Services was founded on a simple conviction: that adults with complex needs in Tennessee deserve better than being turned away or passed from provider to provider. Since 2018, we've built our homes, our team, and our clinical model around that belief — quietly, and without needing our names on the door. What matters is not who we are. To provide a true home for adults with medically complex conditions within DDA guidelines — where residents receive professional, precise care from every member of our team, and where they can grow, learn, and build experiences. Residents actively choose their day-to-day activities, with a focus on improved function, independence, and satisfaction, and with safety as the top priority.
We built our clinical model specifically for high-acuity behavioral and medical support, including the ECF CHOICES Group 6 population. Every home operates on a zero-restraint philosophy, supported by smart-home safety technology, biometric medication administration (E-MAR), and routine mock surveys that keep our teams audit-ready every day — not just when a review is scheduled.
We serve adults 18 and older across Middle Tennessee who need residential and community-based support — including people with intellectual and developmental disabilities, medically complex conditions, physical disabilities, traumatic brain injury, dementia, and age-related needs. Every person's care is built around their own Person-Centered Support Plan, because no two journeys look the same.
