Destination Healthcare Services

will serve adults 18 years and older, who have needs for residential and community based services in and around Middle Tennessee areas.

We provide custom care to all individuals based upon their Individual Service Plan. This enables everyone to the ideal process for promotion of independence with our 24/7 care services, our clients will receive around the clock direct care and support.

Services will be carried out in a single-family residence, serving up to three individuals.

We offer a home-like setting for people who need supportive services in a community setting. Destination Healthcare Services, will provide housing and care options for people who need skilled, medical and community life services. The following services are also provided but not limited to:

Fully-Private Room accommodations
Home-cooked meals and snacks daily
Varying levels of assistance with daily living activities, such as toileting, bathing, dressing, and grooming
Health services and medication management
Organized recreational activities
Personal and/or non-personal laundry services; housekeeping
Social services and religious activities if applicable
Transportation arrangements for all appointments and recreational activities
Wellness programs
24-hour caregiving services
