Destination Healthcare Services provides medical residential care facilities for Individuals with Intellectual Disablities (under 1915c waiver, and ICF choices currently) at homes that provide 24/7 support. Our homes maintain individual physical, intellectual, social, and emotional capabilities of people supported. Our homes are currently located in Davidson County, of Tennessee. Our homes meet all applicable federal, and state requirements for the individuals we service. 

​ Destination Healthcare services will review each individual Support Plan, to determine best overall care practices. This will allow individuals the best opportunity of care, and support that can be provided within each home. Our services provide personalized care, self-help training, communication and socialization skills, language development, sensory stimulation and motor skills. 

Destination Healthcare Services will maintain all responsibility for ensuring the daily health, safety, and welfare of all people supported.
