RESPONSIBILITIES of the Direct Support Professionals
This position is responsible for providing direct support, positive direction, and assistance to individuals in accordance with program policies and procedures; and personal outcome measures.
• Provides supports to people served by assisting in the development and implementation of all Support Plans, including spiritual life supports, assisting people in achieving their personal goals and desires, providing interaction and choices for activities that support a meaningful day, and supporting people to be independent in activities of daily living skills including personal care, cooking, cleaning, and other living skills as stated in the person’s individual support plan.
• Assists with information gathering for Personal Outcome Measures, and other assessments;
• Assists people with maintaining a clean, safe, and orderly home by following safety and infection control procedures; educates people about maintaining homes;
• Maintains and expands skills and knowledge relevant to providing high quality support and services. Obtains and maintains all required certifications and trainings and applies the information to support people served;
• Maintains open communication and cooperative work effort with Lead DSP and other staff to assure quality and continuity of support to people served

RESPONSIBILITIES:

➢ The Quality Assurance Specialist will work directly with all departments to support the goals in providing quality services to the people supported by the agency.
➢ Serves as the Incident Management Coordinator and reporting allegations of abuse, neglect and exploitation will be the responsibility of the Quality Assurance Specialist.
➢ Conduct quality operational reviews and internal investigations of staff misconduct;
➢ Coordinate implementation of training programs including conducting incident reporting training
➢ Monitor compliance with regulations, licensing requirements
➢ Develop and implement operations education and training on the agency’s documentation system,
➢ Monitor the documentation of daily shift notes entries and use of the electronic documentation system and internal operating practices.

➢ Complete internal reviews and provide operations with specific results of the reviews including recommended improvements and training for operations to implement the recommendations.
➢ Assist with corrective actions and serve as the Quality Improvement liaison and communication funnel between the agency and Regional state office.
➢ Assist with referrals and intake activities for new people in service by reviewing intake information for accuracy
➢ Conduct home visits, announced and unannounced to monitor service provisions, conduct quality checks, for systems, including medication administration.
EXPERIENCE: Must be 18 years or older with knowledge of state of Tennessee DIDD rules and regulations; a minimum of 2 years’ experience working as a Direct Support Professional for people with IDD. Experience in a community residential setting preferred. Supervisory experience preferred. Excellent written, verbal and organizational skills required for ongoing communication with people supported, agency staff and community agencies.

RESPONSIBILITIES:
Essential job duties for this position include the following items. Other duties must be performed as assigned or required
• Provide general attention to the non-medical needs of the client(s) following an established Service Plan/Care Plan. This may include personal hygiene, toileting, bathing, oral care, hair, and/or skincare
• Assist client(s) in and out of bed, excluding the use of mechanical lifting equipment unless trained and documented as competent
• Assist client(s) with walking, including the use of walkers and wheelchairs, when applicable
• Provide meal planning, preparation, and feeding as required
• Provide medication reminders
• Remind clients to complete prescribed exercises when instructed by the appropriate health care professional
• Remain in client vicinity at all times during shift and report any unusual incidents and act quickly in cases of emergency
• Utilize the Destination Healthcare services required time management system for Clock-in and Clock-out procedures when reporting for work and when clocking out from work
• Document services delivered in accordance with Destination Healthcare Services Home Care policies and procedures
• Record and report changes in the client’s physical condition, behavior, or appearance to the supervisor
• Report any unusual incidents and act quickly in cases of emergency
• Adhere to HIPAA regulations
• Provide general companion care services, and homemaker services
• Participate as requested in the quality assurance reviews and evaluations of Destination Healthcare Services
• Stay current with changing technology, including software programs
• Uphold, support, and promote all company policies and procedures
QUALIFICATIONS:
To perform this job successfully, the individual in this position must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the minimum knowledge, skills, abilities, certifications, and educational experience required.
EDUCATION/EXPERIENCE
• High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience
• Ability to effectively interact with clients, families, and Destination Healthcare Services team
• Personal care Training Program Certification and Competency
CERTIFICATIONS, LICENSURE, & REGISTRATION
• State required certifications may be required
• Must possess and maintain a valid Driver’s License and maintain adequate auto insurance
LANGUAGE SKILLS
• Ability to read and understand short instructions, correspondence, and memos
• Ability to effectively communicate through written and verbal language
• Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization

The work environment is in an office and/or indoors in a resident’s home and may contain a potential exposure to illness and/or body fluids. EXPERIENCE: Must be 18 years or older with knowledge of state of Tennessee DIDD rules and regulations; a minimum of 2 years’ experience working as a Direct Support Professional for people with IDD. Experience in a community residential setting preferred. Supervisory experience preferred. Excellent written, verbal and organizational skills required for ongoing communication with people supported, agency staff and community agencies.
EDUCATION: HS Diploma or GED required, Associates Degree or college courses in Nursing

Job Requirements
• Graduate from an accredited nursing program
• Valid state Licensed Practical Nurse certification
• Valid driver's license and auto insurance
• Ability to lift between 20 and 50 pounds and provide CPR after certification
• Ability to pass a background check and drug screen

Job Responsibilities:
• Provide prescribed medical treatment and personal care and services to persons with disabilities by performing the following duties:
• Assists RN and professional staff in delivering quality nursing services to persons supported.
• Administers specified medications. Removes expired medications from circulation and access.
• Takes historical information and records persons' supported vital signs.
• Observes person's supported mannerisms and reports adverse reactions to medication or treatment as needed.
• Advocates for individuals with IDD
• Participates as an intricate part of the interdisciplinary team for the people we support
